The procedure

Blepharoplasty is usually performed under local anaesthesia with sedation. For upper lids, excess skin and fat are removed through an incision hidden in the natural crease; for lower lids, the approach may be through the inside of the lid, leaving no external scar. Fine sutures close the incisions. The procedure typically takes one to two hours. You will rest with cool compresses afterward before going home the same day with detailed aftercare.

Recovery

Swelling and bruising around the eyes are expected and look worse than they feel, settling over one to two weeks. Cold compresses, head elevation, and prescribed ointment support healing. Stitches are removed within days. Light sensitivity and watery eyes are common early on. Final refinement continues for months, and sun protection helps scars fade as well as possible. Most patients feel comfortable returning to social activities within two weeks.

Risks and limitations — stated honestly

  • Dry eyes and irritation: eyelid surgery can temporarily worsen dryness or cause a gritty sensation. Pre-existing dry eye is evaluated beforehand, and lubricants are part of aftercare.
  • Difficulty closing the eyes: removing too much skin can impair lid closure, which is why conservative planning matters. Temporary tightness is common; lasting problems are uncommon with careful technique.
  • Asymmetry: the two eyes are never perfectly identical, and minor differences in healing can occur. Significant asymmetry is uncommon but is discussed as a possibility.
  • Visible scarring: upper-lid scars usually hide well in the crease, but individual healing varies, and lower-lid incisions can occasionally heal visibly. Scar maturation takes months.

What is the difference between upper and lower blepharoplasty?

Upper blepharoplasty removes hooding skin (and sometimes fat) from the upper lids, often improving both appearance and vision. Lower blepharoplasty addresses bags and puffiness beneath the eyes, frequently by repositioning fat to smooth the lid-cheek junction. Many patients need only one; some benefit from both. Will blepharoplasty fix my dark circles?

It depends on their cause. Circles caused by shadowing from bags or hollows often improve when the contour is smoothed.
